West Kootenay Health Forum ~ Dental Prevention Through Topical Fluorides ‘By Dr. D. E. Marquis Regional Dental Consuitant to’ West Keoteney Health Ll Teeth that have a trace of fluoride Incorporated in- to the crystaline structure of the dental cnamel are more resistant to dental de- layers of exposed tooth surfaces after tooth eruption by the topical (sur- face) application of fluoride. containing agents. The var- fous methods of topical fluoride applications also result in a significant re- duction in dental decay pre- valence. And topical fluor- ide applications on teeth which have benefitted from fluoridation of water will make them even more dis- ease resistant. ‘he most common daily use of a topical flu- oride agent is fluoride-con- taining toothpaste, for ex- ample “Crest” (stannous fluoride) or “Colgate MFP” (sodium monofluorophate.) These “therapeutic” tooth- pastes not only help clean the teeth but also have a benefit a minimum number trained perronnel. School dental program have conducted “Brush-Ins” in which a whole classroom of children receive toothbrush- ing instructions followed by group self-application of ecially formulated fluor- ide toothpaste. These “Brush-Ins” may con ducted annually or quarter- ly under the supervision of teachers, nurses, and den- tal personnel, Each group “Brush-In" requires approx- imately an hour to com- plete. Dental decay reduc- tions of greater than 30 per cent have been reported. The advantage of “Brush- In" programs is that in- struction of oral home-care can be combined with top- teal fluoride applications for large groups of child- ren in a short working per- jod. Cost of conducting such programs is low in terms of money and time required when compared to the sav- ing from reduced dental de- cay. x A similar method of self-applied fluoride egent makes use of fluoride mouthrinse at weekly in- tervals by groups of child- ren. This simplified method ed bi of ef of topical fluoride treatment requires only a few minut- es of school time each week and costs less than a $1 per child. This method, recently used for 20 months in Portland, Oregon, schools, resulted in 8 gnificant reductions in decayed permanent teeth in the order of 25 to 50 per cent in varlous groups. It Is increasingly ap- parent that a major area of tooth decay can be prevent- ed by. combined use of sy- stemic and topical fluoride therapy. Fluoridation of all L, J. The teeth are thor- oughly cleaned using an ab- rasive compound followed immediately by surface ap- plication of a topical fluo- tide solution or gel con- taining stannous fluoride, sodium fluoride or fluoro- phosphate. This treatment is repeated annually or semi-ancually resulting in a reduction in decay of 30 to 60 per cent. The advantage of this method of topical fluoride application is its relative simplicity, _effec- tiveness in control of den- tal decay, optional use, and convenience.
