available, health care cost an estimated $51 billion. That is
more than double the almost $23-billion pricetag 10 years
ago and almost six times the amount in 1973, the year af.

Aside from inflation, a number of forces are driving
up costs.

Doctorsand hospitals have become increasingly
reliant on complex medical procedures backed up by big-
buck technology. And consumers, who demand top-
quality care, expect the government to pay.

That creates conflicts for provincial governments,
which already spend one-third of their total budgets on
health care.

Deficit-conscious but voter-sensitive, they are forced
into a push-pull of the purse-strings as they struggle to
contain climbing costs without eroding services.

But overall funding of the system is not a problem,
says Robert Evans, a health economist at the University
of British Columbia.

Since 1980, health spending has remained between
seven and nine per cent of the gross national product, the
total value of goods and services produced by Canadians

“The fact that the share of (health spending) has
remained relatively stable says the thing is not out of con-
trol,”" Evans says. ‘‘What is ‘happening is it's becoming
increasingly difficult to control.”

Evans and other experts agree that such problems as
long surgical waiting listS are an early symptom of a
potentially serious illness — one that needs urgent treat
ment to keep it from reaching critical condition

But ‘‘the Canadian propensity for self
congratulation about our. health-care system may be blin-
ding us’ to the need for change, says Dr. David Naylor,
an internist at Sunnybrook Medical Centre in Toronto.

**We have a very unmanaged system where funds arc
simply dribbled in and in that situation, it’s the
squeak y-wheel rule that prevails,”’ says Naylor.

“The loudest complaining (health-care) providers
make what appears to be the best case of the moment, win
the day, and get the dollars for the new clinical unit or the
new beds or the new operating room

“And we have nothing in the way of regional, let
alone provincial, let alone national, co-ordination of
priorities and decision-making.”

owners,”” with a say in how the system is operated

Ontario has six of the comprehensives in the
plasining stages — in Toronto, Trenton, Kingston,
Ouawa, Wawa and the Rainy River district

Given lump-sum funding from the province, the
organization decides where the money will be spent
“*to best meet the needs of their population,” says
Marriott, who heads the program

British Columbia is also looking at more efficient
ways of delivering care.

‘Since 1988, hospitals in the Victoria area have
banded together to co-ordinate services with other care
providers and social agencies

The Victoria Health Project has links with
**wellmess”” centres aimed at teaching preventive health
care; an adult day-care centre which operates a
“bridging” service to ease the transition from hospital
to home; and medical “‘quick response teams” which
assess whether a patient can be treated in the home
rather than an expensive hospital bed.

New Brunswick continues to expand its extra
mural hospital, a program begun in the early 1980s

“It’s basically a hospital without walls,"” says
David Gibbs of the Health and Community Services
Department. “‘In other words, you provide hospital-
type services to patients in their own home.”

About 70 per cent of New Brunswick is covered
by home-care services administeredeby a central extra-

mural hospital board. The program has several
benefits, says Gibbs, including reducing the need for
adding expensive hospital beds, getting hospital
patients discharged earlier and postponing entry into
long-term care facilities

“The cost per patient-care is probably a tenth of
what traditional hospital costs are, because you don’t
have all the overhead of the building, feeding the
patient, the cleaning and all the various components,””
says Gibbs.

**As well, there’s better service to people. You're
treating them at home, and that’s where most people
would prefer to be.”"

New Brunswick has also limited doctors’
payments in a bid to slow soaring health costs, which
rose to $906 million from $255 million between 1979
and 1989. Costs this year are expected to reach $1.1
billion, including a cap of $186.5 million for doctors
treating the province's 750,000 residents

While cost-containment is a major factor behind
the moves, Gibbs says they are also aimed at
improving delivery of services

**The other avenue that comes in is doing more in
the area of promoting good health and preventing
disease,"* he says. **If fewer people are ill from
lifestyle-related diseases, then there should be less
impact on the system

directions, all across the country."
