But no one knows whether Ribavirin, a radically new anti-viral drug, is of any use in fighting AIDS — acquired immune deficiency syndrome — or if it may end up doing more harm than good. In fact, so many Americans have used Ribavirin that a iary of the ps that i d the drug — ICN Pharmaceuticals Inc. of California — has had trouble finding enough people who haven't taken it so that scientific studies about its effects can be done, says Dr. Michael Davis of Health and Welfare Canada. “One has to be very cautjous in using such drugs,” says Dr. Norbert Gilmore, a leading Canadian expert on AIDS. “It's one thing to be dying from AIDS and jump at the chance of taking something that may prolong your life a bit,” of the AIDS virus to reproduce itself and has been found to prolong victims’ lives. Trials will soon be conducted on thousands of AIDS patients in the United States and Canada to study its effects. Gilmore thinks Ribavirin, which inhibits the growth of AIDS and other viruses in test tubes, is worth studying further. “Any drug that has any possibility (of fighting AIDS) has to be looked at — and rather dispassionately.” TREATS DISEASES The makers of Ribavirin claim in their ional In its publicity drives, ICN clearly leaves the impression that Ribavirin might work against AIDS, herpes, influenza and other diseases. When the U.S. Food and Drug Administration approval early this year to market Ribavirin for # purpose, ICN captured headlines by playing up the AIDS connection. ‘The federal agency demanded a series of changes in ICN’s fat press brochure because of a number of misleading material that it is a “miracle” drug — the first drug that is effective against a wide range of viruses. It is currently sold in some 30 countries as a treatment for everything from measles, shingles, hepatitis and influenza to herpes, Lassa fever and AIDS. Many of those countries are in the Third World where there are few if any testing requirements for new drugs, says Davis, chief of the federal Health Department's Gilmore said. “It's P| ly different for sor who has antibodies to the AIDS virus and may never get the disease to experiment with a drug that has not been fully tested.” MILLIONS EXPOSED Up to two million North Americans are thought to have AIDS antibodies, meaning they have been exposed to the virus but have no symptoms of the disease. And scientists have no idea how many of those people will go on to get AIDS, for which there is no known cure. “Such people could benefit from them (drugs like Riba- virin); on the other hand, they could get worse,” Gilmore said. In the past, several drugs that have looked promising in laboratory tests later proved disappointing in treating AIDS victims. One new drug that has received attention recently is AZT, developed by the U.S. firm Burroughs Wellcome Co. Although it doesn’t cure the disease, AZT reduces the ability fe and gy division. Ribavirin, which has taken ICN Pharmaceuticals about 20 years and $75 million US to develop, is also being tested on AIDS patients in the Soviet Union. But ICN officials declined to discuss the matter, saying only that “it's a touchy subject.” Yet in Canada and the United States, which have among the world’s toughest regulatory requirements for new drugs, Ribavirin has only received permission to be sold for one narrow purpose — to fight respiratory syncytial virus. RSV, as it is known, “is usually less significant than a cold,” Davis said in a telephone interview from Ottawa. But in some cases it can kill, especially if it strikes infants who have other serious health problems, such as heart disease. Ribavirin is the first drug that has been shown to be effective in treating RSV, independent experts maintain. The number of babies whose lives are threatened by RSV, however, “is a relatively very small population . . . and the company is not going to make a fortune this way,” Davis said. about the drug's and its effect on AIDS. The conipany promptly Faye Peterson, the agency's public affairs official, said in a telephone interview from Washington. After Canadian approval, ICN again made comparable statements about Ribavirin as a possible anti-AIDS drug ata news conference recently in Montreal. NOTHING WRONG? ICN officials said they don't think there is anything wrong with selling the drug in other countries to fight such diseases as AIDS, herpes and influenza, even though the drug is not approved for those purposes in countries with stringent controls, such as Canada and the United States. They also think there’s nothing wrong with talking about the ial of using Ribavirin to fight such di “We're doing work in these areas (AIDS and herpes) - - - 80 it’s quite legitimate to talk about it,” said Dr. Bryant Rossiter, ICN vice-president. “This is the opening ofa chapter in a new era of chemotherapy.” But others disagree, saying it is too soon to make such sweeping claims. No false hope should be held out, especially to AIDS victims, when the data about Ribavirin's effective- ness and safety aren't fully known, they say. “They had great difficulty in proving that this drug does anything at all,” Davis said. “The sort of data they've presented have always been somewhat deficient.” Said Gilmore, the expert on AIDS: “A drug should only be used when it is known to be safe and efficacious. And we still don't know about his specific drug.” ‘sy0g J0ANS,, PUR eewieND eNUM. ‘s6v0s pemeey 19) eet IE Premenstrual AL ~)( wa 10604 pus eves (©) 10482) BUM YON ey) pue Koq ownyeg eyneyeeg ( 2 semoo JO pence vewomerjiod 1060180) ‘seers YIP - LZLL dOHS JONVENSNI dO1S INO BNOA ONVUASNI JOHOD ‘dweo volesueouos 1" fu. yz '20Q ‘Aopseupeyy 0; yBnosy) g “2eQ ‘Aopsuny) 406014805 AW OYqUINIOD SFI 87TS-S9E MOA 404 BUOY O YIM. The study found often mild thyroid deficiencies in 94 per cent of the victims of premenstrual syndrome who were tested. Those who took thyroid pills “reported com plete relief from the symp. toms of PMS.” However, several experts said they doubt that under active thyroids what's known as hypothyroidism contribute to premenstrual syndrome for most victims of the common disorder. The apparent link between the thyroid gland and pre- menstrual syndrome was dis. covered by Dr. Nora Bray shaw of the Biopsychiatry Centre in Watchung, N.J. The results of he rprelim inary experiments were pub- lished as a letter in the New England Journal of Medicine. In an interview, she said she has found more than 300 women “where the correl. ation is absolutely profound” between premenstrual syn drome and underactive thy roids, and said she has used thyroid pills routinely to treat them BOSTON (AP) — Thousands of Americans are suffering blindness and facial spasms because an experi- mental drug has been shelved by lack of liability insurance, Harvard Medical School professors say. “To have the drug unavailable is a tragedy.” said Dr. Gary Borodic, adding that many patients have been travelling to Canada and England, where the drug is still sold “These patients are so desperate they will go to virtually any lengths to get the problem cured,” added Dr. Daniel Townsend, who held a news conference with Borodic and three patients. The patients suffer from a rare neurological dis order, blepharospasm, which causes involuntary muscle spasms. The affliction can force eyes shut, contort faces and render victims speechless. The cause of the disorder is unclear by Oculinum, created from the botulism toxin, can give relief to most of the estimated 50,000 victims in the United States, Townsend said. The medication’s maker, San Francisco ophthalmol. U.S. law shelves drug ogist Alan Scott, stopped distributing it last spring to 250 doctors who were using it in clinical trials. Scott said he could not find liability insurance while awaiting final approval from the federal Food and Drug Administration. “I can't use my eyes at all,” said one patient, mary Sousa, 76, as her eyes blinked and watered. “By evening I can do nothing but go to bed with a scarf over my head to cover my eyes. It's miserable.” She said she would assume all liability if she were allowed to continue the treatment. “The (federal approval) application is sitting on somebody's desk while a great many people are suffering intolerably,” said Dr. Thomas Crowe, 73, a radiologist who retired 10 years ago when his involuntary blinking made him unable to work. The FDA has said it cannot comment on drugs under investigation. Scott was unavailable for comment Tuesday. The drug works by paralysing the nerves which are contracting involuntarily. Patients feel the effects within minutes of injection. The suspected pump is a chain-like protein that snakes in and out of cell walls. While it serves some unknown function in normal cells, drug-resistant cells have the protein in unusually high amounts. Further research may someday allow doctors to hinder the pumping opera tion in cancer cells, or even activate it in healthy cells to help them withstand the poisonous onslaught of chemotherapy, scientists said in interviews.
